We are currently working to resolve ongoing issues with our lease and building.

 If you are a registered patient with us, please be assured services are currently unaffected and you will continue to have access to GP services locally. 

Please continue to contact the practice as normal and attend booked appointments.

We will write to you in the coming weeks to share more information about the future arrangements. We are actively exploring options with the help of NHS South West London.

Appointments

You can request an appointment by filling out this simple form. Requests made using the form are dealt with in exactly the same manner and with the same priority as telephoning us.

If you find it difficult to use a computer or a smartphone, you can telephone our reception on 020 8643 3030 and our receptionists will fill out the form on your behalf.

If you wish to change or cancel an appointment, then click here. Select the Admin query option, and the option labelled I need help with something else.

Prescriptions

You can order repeat prescriptions through your usual pharmacy, or request them online.

Test results

You can obtain test results and full access to your medical records online.

Please note that we shall call you if we have any significant concerns about your results.

Musculoskeletal Self-Management

The getUBetter app is provided free of charge by Sutton CCG for patients registered
at Carshalton Fields Surgery. It can be accessed on a Smartphone or on the Web and
guides you day-by-day through a sequence of exercises and tips to help you get better
from a range of new or recurrent musculoskeletal conditions:

• Lower back pain
• Back and leg pain
• Neck Pain
• Shoulder Pain
• Ankle Pain
• Knee Pain
• Soft Tissue Lower Limb
• Hip Pain

You will be referred to the App by one of our GPs or Physio, or you can self-register by
clicking on this link or the App image below.


Please use an email address that is unique to you and select the condition you need
help to manage. You will then be sent an email with all the information you need to
get started.

You can download the getUBetter App to your Smartphone for easiest access or use
the Webapp

If you develop another condition register again with the same email address, select
your new condition and it will be added to your account.

The app will also connect you to your local; treatment, healthcare providers or
support services if needed like Physiotherapy.
