Community Nursing

Community matrons

Community matrons see people in their own homes to help them stay independent and to enjoy the best quality of life. They provide care, advice and support to those patients who have complex long-term conditions such as diabetes, chronic respiratory disease and heart failure.

The community matrons see and assess patients with complex health needs to ensure that they are receiving the correct care to promote independence to maintain quality of life.

District Nursing Service

District nurses provide nursing care to housebound adults over the age of 16 years in their own homes. This service is provided 24 hours a day, 365 days a year.

There are 12 district nurse teams based throughout Sutton and Merton, aligned to general practitioners.

End-of-Life Care Service

The End-of-Life Care Team supports district nursing teams to provide best practice in end-of-life care through the implementation of the end-of-life care tools such as the Gold Standards Framework, Liverpool Care Pathway and Preferred Priorities of Care (Advance Care Planning).

The Macmillan Nurse Consultant in End-of-Life Care supports improvements to end-of-life care in the community setting by:

  • supporting clinicians in primary care and care homes
  • overseeing service improvement in end-of-life care
  • education and training and other workforce development around end-of-life care.

Identifying people who are in the last year of life is crucial to planning ahead and supporting an individual’s preferences around end-of-life care. A Gold Standards Facilitator supports that goal in care homes and a facilitator for primary care is currently being recruited.

The Liverpool Care Pathway Facilitator is implementing a care pathway for people in the final days of life, supporting clinicians in providing best practice.
