Latest news on Carshalton Fields surgery 

As you may have heard, Drs Raj and Kusum Goel (our landlords) are evicting us from the building on Crichton Road. We are pleased to inform you that we shall merge with Wallington Family Practice on 1 July 2024.

You do not need to take any action if you wish to continue to receive care from Dr Carrasco and her team:

Your registration and medical records will automatically transfer to Wallington Family Practice on 1 July 2024.

Wallington Family Practice is a well-established practice that is led by Drs Ahmed, Hossein Ally, Noorani, and Roberts. Dr Carrasco will join the partnership and all the clinicians and staff you are used to seeing at Carshalton Fields will be transferred over. You can still see Dr Carrasco and her team if you wish, but you will also have access to a much larger number of clinicians, including another nine GPs, seven nurses, plus a range of other clinical staff.

Wallington Family Practice is situated in Jubilee Health Centre, Shotfield, SM6 0HY. Jubilee is a modern, purpose-built building and is a significant upgrade over the Crichton Road site. It is just over a mile away by road and is well served by public transport. The route 154 bus route runs directly from the Crichton Road bus stop to Shotfield. There is plenty of parking available, including dedicated blue badge parking bays directly in front of the building. In addition to the additional services that are provided by Wallington Family Practice, at the Jubilee HC you can also get x-rays, blood tests and MSK services. There is also a pharmacy on the ground floor.

Patient Triage

We have now gone live with our new online consultation service-Patient Triage. This is a great way for you to contact our practice for non-urgent medical or admin requests. It is very simple to fill out so please do give it a go if you need to contact us. Although the character count is limited on the form, please try to give as much specific and concise information as possible. We really think it is easier for most people than waiting on hold to speak to a receptionist but would love to hear your feedback. If you do not have access to the internet or find it difficult to use a computer or smartphone you can call reception instead.

We have updated our Privacy Notice

  • This practice handles medical records according to the laws on data protection and confidentiality.
  • We share medical records with health professionals who are involved in providing you with care and treatment. This is on a need to know basis and event by event.
  • Some of your data is automatically copied to the Shared Care Summary Record
  • We may share some of your data with local out of hours / urgent or emergency care service
  • Data about you is used to manage national screening campaigns such as Flu, Cervical cytology and Diabetes prevention.
  • Data about you, usually de-identified, is used to manage the NHS and make payments.
  • We share information when the law requires us to do, for instance when we are inspected or reporting certain illnesses or safeguarding vulnerable people.
  • Your data is used to check the quality of care provided by the NHS.
  • We may also share medical records for medical research
